An otaku is someone who's so obsessed with something that they hardly venture out of their house or stray from their obsession for more than a few minutes. You could also say that Otaku is a term akin to Hikkikomori, due to the tendency to stay inside and not stray from their hobbies or obsessions. The insane bit is pretty self-explanatory, and defines her erratic and strange behaviour.
